Bring a photo ID, your insurance card, a current medication list, and any referral paperwork required by your insurance plan. If you've already had X-rays, an MRI, CT scan, or other imaging related to the problem, bring the images and reports if they're available, along with any relevant records from previous providers. Having prior imaging and records available can help Dr. Mustovich understand what treatment you've already received and may help avoid unnecessary repeat testing.
You do not need a referral to schedule an appointment with Dr. Mustovich unless your insurance plan requires one. Some insurance plans require a referral from your primary care provider before seeing an orthopedic specialist, so it's a good idea to check your plan's requirements before your visit.
